Scholarships

2024 Foundation Scholarship Winners

Each year students pursuing careers in health care are the recipients of scholarships awarded by the Grundy County Memorial Hospital Foundation, thanks to the generosity of local donors. Scholarship awards are made each August.  The 2025 Application and reference forms are now available. Completed applications must be received by May 31, 2025, 4:30 p.m.

2025 Health Care Career Scholarship Application 

The following are the eligibility requirements and approved programs for a GCMH Foundation Healthcare Career Scholarship:

Eligibility:

  • Student is a high school senior enrolled in a post-high school degree program leading to clinical licensure or pursuing a career in health care, or a current student enrolled in an accredited health care career program leading to clinical licensure or a health care leadership role
  • Student must have completed at least one semester of their official, accredited health care program, or be a licensed health care professional pursuing an advanced degree in health care to be considered for the Kliebenstein, Stout, Ware or Yoder Scholarships. Student must be a high school senior enrolled in an eligible health care career program for the fall semester OR in their first or second year of post-secondary studies to be eligible to be awarded a GCMH Foundation Scholarship.  
  • Student must be enrolled at least part time for either undergraduate (with a minimum of 6 credit hours) or graduate programs (with a minimum of 4 credit hours)
  • Student must reside in, or have graduated from a high school in, the Grundy County Memorial Hospital service area of Butler, Grundy, Hardin, and Tama counties or currently work at Grundy County Memorial Hospital   
  • Student must have a cumulative post-secondary GPA maintained at 2.5 or higher
Traditional and nontraditional students are encouraged to apply.  Previous recipients and applicants of the GCMH Foundation Health Care Career Scholarships are encouraged to apply.

Approved Programs:

- Nursing (LPN, ADN, RN, BSN, MSN, DNP)

Nurse Anesthetist

- Doctor of Medicine (MD)

- Doctor of Osteopathic Medicine (DO)

- Dietetics

- Laboratory Sciences

- Occupational Therapist or Occ. Therapist Assistant

- Paramedic

- Pharmacist

- Physical, Occupational, or Speech Therapist or PT or OT Assistant

- Physician's Assistant

- Radiological Sciences

- Respiratory Therapy 

- Healthcare management/administration


Eleven or more GCMH Foundation Health Care Career Scholarships will be available in 2025: 
- Charles L. Ware Memorial Scholarship ($1,000)
- Don and Susie Kliebenstein Family Health Care Career Scholarship ($1,500)
- Irene A Stout Health Care Career Scholarship, for nursing ($1,500)
- Sara Lee Yoder Health Care Career Scholarships ($1,500)
- Student is a high school senior enrolled in a post-high school degree program leading to clinical licensure or pursuing a career in health care for the fall semester and students completing their first and second years in a health care career field of study.
